Library Instruction Requests

This form is for HACC faculty to request library instruction for HACC classes. Librarians are available to conduct sessions tailored to your individual class needs. Please submit this form a minimum of one week prior to your requested date of instruction. Please submit one form for each class. A librarian will respond to confirm your class session. Faculty are expected to attend all library instruction sessions that they have requested unless other arrangements have been made in advance.

Please use ONE form per CRN you are requesting instruction for. This will help us manage our instruction loads as well as participate in college-wide assessments. You may request multiple sessions per CRN on a single form, but if you are teaching multiple sections please submit a different form for each section.
