Library Instruction Requests

Use this form to request library instruction for your class. Librarians are available to teach sessions tailored to your class learning outcomes. Please submit one form for each CRN at least one week before your desired date of instruction. A faculty librarian will respond to confirm the instruction.

Please use ONE form per CRN you are requesting instruction for. This will help us manage our instruction loads as well as participate in college-wide assessments. You may request multiple sessions per CRN on a single form, but if you are teaching multiple sections please submit a different form for each section.
